Jay L. Levin, M.D. – Orthopedic and Spinal Surgeon
Dr. Jay Levin attended Northwestern University, Evanston, IL, followed by graduating from Rush Medical College, Chicago, IL, in 1980. He then attended an orthopedic surgical residency at Rush Presbyterian St. Luke’s Medical Center until 1985. Among other honors, he was-”Intern of the Year.” Pursuing additional training in spinal surgery, he completed a fellowship in adult and pediatric spinal surgery at Rush Presbyterian St. Luke’s Medical Center and The Shriner’s Hospital for Crippled Children under the direction of renowned spinal surgeon, Dr. Ronald DeWald. Dr. Levin has been selected to Castle Connolly’s List of America’s Top Doctors, Chicago metro Area. He is board certified by the American Board of Orthopedic Surgery. Dr. Levin is a founding member of Adult & Pediatric Orthopedics, S.C.

David M. Zoellick, M.D. – Orthopedic Surgeon
Dr. David Zoellick graduated with honors from DePauw University, Greencastle, IN, in 1983, and he received his Medical Degree from Northwestern University, Evanston, IL, 1987. He interned at Northwestern University McGaw Medical Center in the Department of Orthopedic Surgery. Dr. Zoellick was a staff orthopedic surgeon at the US Naval Hospital, Guam, for the US Navy from 1992-1994. He also resided on staff at the US Naval Hospital, Great Lakes, IL. He has extensive experience in sports medicine, trauma, and has been involved in treating athletes from local girls travel softball and travel basketball teams. He also played for the Guam Naval Air Station championship basketball team. As a resident at Northwestern, training included physicals of perspective NBA players at the NBA combines as well as covering Chicago Cubs baseball games, Evanston High School football games, and Chicago Public League soccer tournaments. Currently he is the team physician for Mundelein High School, Mundelein, IL. He was head of Orthopedic Surgery at Condell Medical Center, Libertyville, IL from 2004-2008.

David Schafer, M.D. – Orthopedic Surgeon
Dr. Schafer graduated cum laude from Georgetown University in 1995 where he also received a varsity letter in football. He attended Northwestern University Medical School and graduated with honors. He proceeded to complete his orthopedic surgery residency training at Northwestern University McGaw Medical Center in 2004. With a strong interest in sports medicine and the latest arthroscopic techniques, he completed a fellowship specialty training in sports medicine from the esteemed Kerlan-Jobe Orthopedic Clinic in Los Angeles. During that time, he served as assistant team physician for several professional teams including the Los Angeles Dodgers, Lakers, and Kings. He also learned the most advanced arthroscopic techniques for treating shoulder and elbow pathology while honing his skills in all other aspects of orthopedics. Dr. Schafer has been in practice in the Chicago area since 2005 and has extensive experience in trauma, sports medicine, and joint replacement surgery, including minimally invasive partial knee, shoulder, and elbow replacement surgery.

Dr. Martin Lanoff received his medical degree from Chicago Medical School. He interned at Cabrini Medical Center in the Department of General Surgery then he completed a residency in Physical Medicine and Rehabilitation at Hines VA Hospital. Dr. Lanoff is board certified by the American Board of Physical Medicine and Rehabilitation. He is a Diplomate of the American Board of Pain Medicine. Dr. Lanoff has held national positions in the American Academy of Physical Medicine and Rehabilitation. Dr. Lanoff has been selected to Castle Connolly’s List of America’s Top Doctors, Chicago Metro Area. He is a clinical assistant professor in the department of Rehabilitation Medicine at the University of Health Science, Chicago Medical School. Dr. Lanoff’s special interests are rehabilitation of industrial and sports related injuries, musculoskeletal disorders, electrodiagnosis and spinal injection procedures.
